1. Water parameters. (ammonia, nitrate, nitrite, PH, temp', Hardness etc)

I don't have exact numbers, but I do go frequently to get the water tested - this last run they said all the parameters were fine.
Temperature is usually between 75-79.
I do use bottled SPRING water, the water out of the faucet here is not something I would even drink let alone let my betta live in it. It's reeks of chlorine, so i wouldn't trust it even with purifiers.

2. A full description of the fishes symptoms.
He seems to have a whole cocktail of issues. About two-three weeks ago he started to hide behind the tank filter and refusing to eat. He also had white patches on his head, treated with bettafix and that went away. However, he is still laying around and not eating. This morning, I found him laying sideways on the bottom of the tank. He used to be really active and would meet me when it was time to feed him.
I did some further investigating when I did a water change today - it looks like fin rot now AND some sort of red sore on his gill.
The most concerning thing for me is that he tends to stay at the surface of the water, constantly gasping at air.
I purchased bloodworms today to see if maybe he just needed something else to eat - but he would just push them out of the way to grab at air.

3. How often you do water changes and how much.

I've been changing every 3 days to try to remedy any issues with the water.
Prior to his illness, it was weekly 25% water changes at least, with a bi-weekly complete water change.

4. Any chemicals and treatments you add to the water.
Currently switched to Melafix this morning.

5. What tank mates are in the tank.

None. No live plants either.

6. Tank size.
1.5 US gallons

7. Finally Have you recently added any new fish?

No.

AHHH. Went to look at him closer and he is quickly disappearing! His fins have been slowly deteriorating until todays water change. I'm super worried. :(
(Oddly enough, he looks to be a bit more active in his tank now..)

He looks pretty fat. Id cut back on feeding for a while until his tummy size goes down. Are you able to get any antibiotic meds? Id treat him for a bacterial infection if his fins are deteriorating and you are seeing sore spots on the fish. Also lowering tank temp a degree or 2 C would help slow down the bacterial growth. and lastly, keep up with the waterchanges!

He hasn't eaten in two weeks. He seemed to be doing really well yesterday! He was finally swimming around.
This morning he has me scared. He was resting on the filter, and he is still there when I came back from work. :no:
I can't locally get any antibiotics - is there something I should order?
The melafix is not working. or maybe even making him worse.
