I started a 135 gallon tank in Sept, cycled it and had the water tested numerous times. Since then I have had a consistent problem with fish dieing, they pass one at a time about one per week or every second week.
Different types including gourami's platties and bettas. I test regularly and change water weekly but the problem continues.

Any thoughts or suggestions would be appreciated.
 
What type of bettas? If they are splendens they are not community fish and you should expect losses - especially as you have more than one.
Bettas and gouramis should not be kept together
What is the GH of your water (general hardness)? Bettas and gouramis require soft water and platies require hard water. Platies won't survive long in soft water.

Not meaning to sound negative but it sounds as if you were given bad advice by your fish store and should review your stocking.
 
Please post your water parameters (ammonia, nitrate, nitrite, pH, GH, KH, temperature). Do you have a heater and filter in your tank? Are you using a water conditioner when adding new water? Can you tell us how much you are feeding and what types of foods?
