My New Betta Keeps Getting Sick

Hi everyone
I have a betta that I have had since March and in this short time he has suffered velvet disease (in which he almost died) and has basically just recovered from this. I still have multicure in his water to beat the last of the disease but now I am noticing he has a tattered back fin which I understand to be fin rot. I am a little distressed about how hard it seems to be to keep this fish from suffering from some medical issue and would really appreciate some tips about how to keep him healthy

What size is his tank? Does it have a heater? What is the temperature?
Does it have a filter? If so, is it cycled? What are the water parameters (pH, ammonia, nitrite, nitrates)?
What is your water change schedule? (How often & how much) What do you feed him? (How often & how much)
 
Are there any other tankmates? If so, what are they and how many are there?
 
Can you get a picture? You can post pictures by going to 'More Reply Options' and attaching file at the bottom. If that doesn't work you can go to a photo sharing website such as photo bucket and link it here.

If your betta is getting sick this much it may be that you have bad  water quality.
Ninjouzata pretty much summed up what info we need, but we may need a pic of it, as it might not be fin rot, maybe just fin wear / tear?
 
Basically, just answer all of ninjs' questions and then we will be able to assist you further. But right now in afraid we don't know enough.
