Has Anyone Lost A Fish Due To Being Trapped In The Filter?

I came home from work last night and found one of my Long Fin Widows stuck in the bottom part of the filter dead, :( how he got caught in there i have no idea, i was gutted as i've only had him for 3 weeks. Has anyone else had this happen to them before, such a horrible way to die but i have no idea how to avoid it happening again although i guess it's pretty rare?
 
Most likely, he died or was dying and got stuck to it. It happens a lot. It wouldn't have gotten stuck if it was totally healthy. Normally, the fish would be so weak, it would get sucked onto it and couldn't get off. Or he was dead and just got sucked onto it. :(

Totally agree with all the above. Although water is expelled from the filter at quite a pace this is because it goes through a spout. The rate in which it is sucked into the intake is very gentle due to the amount of slots. If you allow sinking food to sink close to the intake you will see how gentle it is. I have had the odd fry in my filter but again this is because they swim into it thinking it's shelter from the biggun's.....wrong!
 
I lost a couple of juvie angels to the intake on a Fluval 304 - not no one was sick or anything and I hadn't had any losses in the tank before that - so I suspect it was the filter... the above posts make a good point too - but i can't think of why my fish would have just died like that
 
I had a little tiger loach manage to get inbetween the outlet and the casing of jewel internal filter, god knows how they manage it, I have a little bit of plastic to make sure theres no gap at all now.
 
i once losted one of my clown loach, couldn't find it anywhere. it wasn't until i tried to clean the filter and found it was stuck between the sponges. manage to get it out only for it to disappear again the next day, so looked in the filter again, sure enough it was there again! it didn't look stress or anything so i just let him be now. now all my clowns play this game where they all swim against the flow and the one manage to get inside the tube win! the winner come out and they play the game again. i should put a net over the tube, but why spoil the fun for them.
 
I lost a cichlid about a month ago due to him being sucked up into the filter... The filter cover had fallen off while cleaning the tank... I suspect he was probably sick though
