Fish Died Please Help :-(

I came back from work today to find 1 of my rainbow reds pointing mouth down between 1 of the plastic plants.
I immediately took him/her out cnd changed the water. When I looked at it, it was realy white and looked like its eyes were missing. I tested the water before i changed it and everything seemed to be spot on. Water temperature was fine and they were fed once a day.
In the tank I had 2 rainbow reds (now only 1), 6 neon tetras, 2 clown loaches and a plec gibbieps (sp).
There are 2 filter systems as well. On the top of the tank are 2 sponges, corse and fine and then a bio wheel (I think thats what it is). I also have an external pump that has a large sponge and a load of ceramic rings.
What could have caused the Rainbow Red to die? I am quite worried.
Is there anything I should do for the others, or is changing the water ok?
 
It could be a number of things. I wouldnt worry about eyes missing, it seems to be the first thing other fish eat when they find a dead fish.

You could look for excessive slime on the fish, velvet, red blotches, cotton wool type material on the fish etc...

None of the above. The rest of them seem to be fine. How do i check the slime though? They all look the same as the day i got them. Appart from the other rainbow red is more red now than when i got him. Looks like his proper colouring.

I did notice on 1 of the rainbow reds that there seemed to ba quite a long thin white thing comeing from the poope hole thing (dunno the proper name). About half its size. When I flick the glass to make it jump, it just falls away like the poop does.
Would that have anything to do with it?
 
Long stringy white poo can be constipation, internal bacteria infection, to internal parasites.
Check the anus to make sure it not red and inflamed or enlarged.
Try some shelled peas first to flush the fish through.
Was he headstanding.
 
It seemed to be the same colour as the rest of it. Didn't look inflamed or anything.

How big is the tank? What are the water stats (pH, ammonia, nitrite, nitrate, temp)? Some people say their water stats are good but they're not. Any chemicals used around the tank (glade plug-ins, air fresheners, etc)?

The tank is 15 inches wide. 10 inches tall and 8 inches deep.
pH=6.4
KH=0
GH=>6
NO2=0
NO3=25
NH3=0
As far as im aware, thats ok. Or am I wrong?
There are absolutly no chemicals anywhere near the tank. No air fresheners in the room, no sprays anywhere... All thats around the tank are 2 small indoor fountains, fish food, the water treatment stuff in a sealed bottle, the external filter and air pump. Thats it. Some water is changed probably every 3 days and the temperature at the mo is 24.5 degrees centigrade.
All the fish are lively, the neons are all bunched together like they should...
As far as I am aware, all that is fine and there should be no problems. Or am I doing something wrong?
 
If you have Glossolepis incisus, the red rainbow, they prefer a slightly alkaline ph or at least a neutral ph with slightly hard water. Your water is great for the neons, plecs and clowns but not so good for the rainbows. You must have some KH or you would get ph crash. I guess your water is so soft that it is not showing on the test kit. I am not saying that the water is the cause of death, but less than optimal conditions for any fish will cause other health issues, I am not even sure how adaptable those rainbows are to acidic water but all fish do best in the kind of water they evolved to live in.
 
I agree with Liam... Also.. If your tank measurements are 15" x 10" x 8" then I would say you're overstocked... From my calculations - correct me if I'm wrong - I would say that you have a four gallon tank maximum?
