I need some help with my tank and betta fish again. I recently got my tank nice and stable at around ammonia: 0 nitrite: 0 and nitrate around 30-40 (what comes out of the tap). He struggled with a lot of fin rot to the point he had a big streak of red in his anal fin (which followed below his body and then went down towards the back and his ventral fins went completely red and looks so sore, inflamed and broken up. I tried melafix for a while but it did not do anything/much with lots of clean water. Against my better judgement, due to him looking terrible i decided to use Kanaplex in my lovely new cycled tank (due to not having a hospital tank and it not being as harmful as full on kanamycin) I checked the water parameters before every water change in between. After the first load of doses the tank still came out fine (as stats above) but the fish only looked a little better. Annoyed i looked on their website and saw someone had successfully treated two week in a row. I tried this as i know if i leave it, it could become resistant and i found it hard to get meds in the first place.
It was all fine, he got a bit better but the end of the two weeks of doses i stopped. He now had a less obvious streak in his anal and a bit less in his ventral but i have a bad memory but i am sure they are getting worse again. To my horror (yet I was not overly surprised) he turned around and bit about a third of his tail off and even reached his anal one too. Thats when i notice something must be up, he had a bit of colour loss around his mouth so i tested the water and the nitrite level had reached 0.25 but ammonia was still at 0 (sorry i cannot find the nitrate bottle), so i've obviously killed off the nitrite converting bacteria in my bio filter.
I'm now doing 2 50% changes a day to keep it minimal but he doesn't look right. He still has a lighter face than normal and his ventricle fins are still inflamed and so red (his bitten tail is healing with a white film). I have no idea what to try next after i get this filter sorted out.
Could it be persistent fin rot? (it was there before the nitrite) or something else? What other treatment should i do.. after minicycle is over?
As it streaks under the body could it be internal? and it goes all the way up and down of the ventral fins

Fish Symptoms (include full description including lesion, color, location, fish behavior): Red ventral fins and one streak in his anal, his face is slightly lighter (i take a lot of notice due to that is the first to change if something is wrong), he is still darting around the tank and playing in the filter stream. Still eats.
Volume and Frequency of water changes: right now 2 50% a day water changes until minicycle over
Chemical Additives or Media in your tank: Prime only (i have melafix at hand, aquarium salt but that makes him ill, kanaplex, melachite green)
Tank inhabitants: Just him
Recent additions to your tank (living or decoration): one cove that i'm going to take out because he hit a scale the other day
Exposure to chemicals: about 2 weeks of kanaplex a week ago
