He looks dead. He also looks as though he has some abrasions on his side. Remove him and check all of your ornaments and decorations for sharp edges that could have injured him

We know it's dead. They've been in there for about 3 months now so we dont think it's the ornaments. Were thinking it's some sort of disease or something. The ornaments have bee bought from pet shops that are safe for fish so they wouldn't be a problem as the other 3 are perfectly fine

Some times ornaments made for fish still have rough edges that are sharp if the brush up against them. Other members as well as I have had that problem. For instance if he was irritated by water quality and started flashing on a rock that was slightly rough he could have gotten an abrasive wound. What are you water parameters?

your water looks cloudy, check the ammonia, nitrite, nitrate and pH.

do a 75% water change and gravel clean the substrate every day for a week.
make sure nay new water is free of chlorine/ chloramine before it's added to the tank.

clean your filter if it hasn't been done in the last 2 weeks.
wash filter media/ materials in a bucket of tank water and re-use them.

We tested the water straight away and everything is perfect, we checks ammonia and all the rest and it's what it should be

We did a water change like 2 days ago and tested the water today and everything is perfect. When we add water we always add tap safe and my camera isn't very good but the water is clear in real life

Looks like it’s had high ammonia , but could have been a past infection. Fins look damaged. I wouldn’t be surprised a Cory would be sick first. Guppies spend very little time at the bottom where detritus is so avoid some bacteria there. Plecos have virtual body armor and IMO it takes a lot to hurt them.
How big is this tank?

We check the ammonia and everything else every week because we often do water changes, everything was perfect. As for the fins they must of been from the other fish thinking its food because it's not moved. It's a fluval flex tank. We just wanna know how what the red patches are
