How long has the tank been set up for?
How often do you do water changes and how much water do you change?
Do you dechlorinate new tap water before it goes in the tank?
Have you checked the water for ammonia, nitrite, nitrate & pH?
If yes, what were the results in numbers?
How long have you had the fish for?
What other fish are in the tank?
Do you use a "Fish Only" bucket for the tank or do you use any bucket around the house?
Make sure you don't have any cream, oil, grease, perfume, soap, disinfectant or anything else on your hands when working in the tank.
Make sure there are no fumes in the room with the tank. this includes things like hair spray, perfume, deodorant, bug spray, air freshener, cigarette smoke, etc.

If you turn your camera on its side (so it is horizontal) the video will fill up the screen rather than just the middle and you will get better footage.

The fish has past its used by date and I would euthanize it. You can put it in a small container of tank water and add 1 drop of clove oil, gently stir, wait a minute then add another drop. Keep doing this until the fish stops breathing. Then put the container in the freezer and let it freeze. After that bury the frozen water with fish in the garden.
If you don't have clove oil you can give the fish a whack on the head to kill it. If that freaks you out then put it in a small container of water with a lid and just freeze it.

If the fish hasn't died yet and you don't want to euthanize it, try doing a 75% water change and gravel clean the substrate each day for a week. See if it helps.
Make sure any new water is free of chlorine/ chloramine before it is added to the tank.
You can also increase surface turbulence/ aeration to maximise the oxygen in the water. Don't create too much water movement tho, otherwise the fish could get knocked about.
