2 possible clues here...
Powerhead intake. Tangs can be incredibly explorative. Its possible that is swam too close to the intake and was killed. Even strong swimmers like tangs cannot swim away from an intake if they get caught. Being small and young it had no perception of the dangers within an artificial tank. Tangs that have soent more time in captivity are a little more streetwise around powerheads.
Moving the layout. Tangs are among the easiest fish to startle and cause stress too. People call them whitespot magnets but this is basically because the are starteld and get stressed in the confines of a smaller tank.
I feel that either one of these were the most probable cause of the tangs demise
