I certainly agree on the CO2 Booster, this can kill plants when used as directed, and if it should happen to get overdosed it can kill bacteria and fish. This in itself should alert anyone as to the danger of using this product.
I still say the Rainbow Shark is the likely issue here. If you have another tank, remove this fish and you will likely have no further deaths. Of course, make sure it is not going into another tank with fish.
