Black Molly Fish

My black molly of a week liked playing in the tank, last evening he buried his head in the rocks on the bottom of the tank and died, anyone have an idea why?

Are you sure he buried his head in the rocks before he died and didn't get blown there by the filter after dying?

How old is the tank?
What are the dimensions of the tank? And volume?
How was it cycled?
What other fish are in the tank?
When was you last water change and how much was changed?
Anything new added to the aquarium lately? Decorations/medications
