that doesnt sound good. i recently had a betta die who was acting the same way. i actually got him to eat one frozen bloodworm every other day or so. bettamax didnt seem to help him so then i tried using indian almond leaf in his water instead. he started doing better and actually started swimming around some and i thought he was on the way to recovery but he died suddenly the other day. i also suspected it mioght be internal parasites of some kind but he wouldnt eat the medicated food.
sorry to discourage you though. yuor guy might pull through. he looks healthier than mine did. just be diligent (and gentle) with the water changes. keep him in a non-decorated tank (take out all of the gravel/ decorations). i would keep him out of direct sunlight too.
