Any Idea What’s. wrong?

This morning the dang Tetra was swollen again after breakfast. I moved her back to the QT tank to fast her and treat her again. Tonight, she’s gone! Committed suicide. Can’t find her anywhere either. Checked the filter, gravel, plant. I looked with a flashlight everywhere and then my husband did. Nothing...anywhere. I even brought my dog in to sniff around. Still nothing. Moved everything around. It was just her time, I guess. :(
This has happened to me many years ago with two corydoras - they just disappeared from the tank.

We found them months later - mummified, behind the unit the tank was on. They had managed to (both) leap and struggle though a less-than-half inch gap at the back of the tank where the glass cover was away from the edge to allow wires for the heater etc to go through.

It broke my heart. How they must have suffered, poor little things.
