Looks like she has gill flukes. The white edging to her gills and gasping at the surface are typical signs of this.
The white stuff on her body & tail is excess mucous that can be from poor water quality or skin infections.
The slightly raised scales on her back half is probably bacterial or protozoan.
I would try rock salt or swimming pool salt, (aquarium salt is fine) and use 2 heaped tablespoons per 20litres of water. The following day add another 2 heaped tablespoons per 20 litres. Keep the salt level high for a couple of weeks and see how she goes. If there isn't a noticeable difference after a couple of days then try some "Waterlife Protozin".
Don't bother feeding her for a few days and if you do water changes on the tank, make up the water and add some salt to the new water before it goes into the tank.
If she is better in a couple of weeks then do daily 10-15% water changes (for a couple of weeks) to dilute the salt level.
You can try Praziquantel for the gill flukes but see if the salt does anything first. Praziquantel is used to treat tapeworm in dogs and cats. It also kills gills flukes and tapeworms in fish. A company called AquaMaster does Fluke & Tapeworm tablets which contain Praziquantel. Otherwise buy some tapeworm pills from the pet section where animal wormers are sold.
Use 100mg of Praziquantel per 20litres of tank water. Do a 50% water change 2 days later. Repeat the dose and water change a week later.
Make sure any new water going into the tank is free of chlorine and has a similar temperature and PH to the tank.
Remove any carbon from the filters if you haven't already.
