With ammonia-nitrite poisoning, initial symptoms are breathing heavy at the surface. The most aggressive variety of Columnaris will wipe out the tank fast with no early telltale symptoms.

If the cat's paws, which are loaded with bacteria from a litter box, contact the water, that can be a source of badness too.
