    I'm not sure what your plant is but it's normal for aquatic plants to be grown above water (emersed) at the nursery and then when you plant them in your tank the emersed leaves die and aquatic leaves grow back, I'm guessing that is what has happened here.

    I wouldn't worry about it, as long as your fish are healthy you're fine, besides nitrate tests are notoriously inaccurate. Unless you're keeping extremely sensitive species, like discus, the fish won't mind levels of 80-100.

    Definitely diatoms. Normal for new setups and will go away on its own in time. Just manually remove it with a sponge or toothbrush as needed. BNs and otos love the stuff if you've got the space.
