Hi, it looks like he has been bashed in the head. Its all red and raw. The female swordtail has a large white mark on her, which isnt fluffy and looks like a scald or burn. No-one else in the tank is affected. For other inmates see my signiture (its tank 1)

I gtg for a driving lesson now but will post new water test parameters afterwards. Last week the water tested at 0 Ammonia 0 Nitrate and 0 nitrates tho it was almost the next colour band.

I do a 15% each week and feed with freeze dried daphnia and tropical flake. the plec gets algae wafers

Substrate is gravel with a fluval (3 or 4 not sure) doing filtration and the heater is set to 24c.

Tank is planted.

You will need to use anti internal bacteria med, does the white mark look abit greyish in colour, if you don't have scaless fish add salt to the tank or melafix.
 
Salt probably wont make life easy for the plants, I would say some melafix and plenty of clean water would be ok if you watch carefully for secondary infections and fungus etc. You could also try some of the medications for finrot.
