Wounded Dojo Loach

Hey y'all!

Can someone please tell me what's going on with my Dojo? She wasn't like this yesterday. She lives in a 75gal tank with 4 other Dojos and 4 Platys. She is one of my very first OG Dojos that I've had since Aug 8th. Everyone else in the tank are fine.

Parameters - Amon 0, Nitrite 0, Nitrate 5, PH 7.4. Temp 70°. I transferred her to my 5gal quarantine tank that currently houses 3 Platy fry.

I wasn't able to get a pic ID the other side of her but it looks pretty much the same.

It looks so painful
What can I do to help her?

What is the tank temp? How about substrate? These loaches are cooler/coldwater and prefer sandy substrate. My guess it that the irritation is due to substrate and possibly too warm waters.

She died the next day. The temp is 70°. Right now I have gravel. I'm in the process of switching to sand. My other 4 Dojos are fine. I don't think it was the gravel just by the shape of the wound.
I have everything I need to swap out the substrate.
There are still a few things in the house that need to be done that I am unable to do by myself.
