What Plants/supplies would I need for a Tank to Look Like This?

how old is it? mine also died back like that, as long as it keeps growing new leaves it will transition

A couple of months I'd say, it had beautiful big lily pad leaves (it's not tripitata, its the other one) and they all melted away and these tiny little ones started growing...and they're staying small well see what happens

Same thing happened to me with Marsilea crenata. I suspect the two are related.

Get two or more smaller pieces of wood rather than one big one . You can then design caves tunnels etc.
