Very Ill Angelfish

Hello! :)

Tank size: 40 gallons
pH: 6.5-7.3
ammonia: 0 ppm
nitrite: 0 ppm
nitrate: 0 ppm
kH: 4 sometimes goes to 6
gH: 5
tank temp: Unknown. I live in Southeast Asia so the water is constantly warm enough.

Fish Symptoms (include full description including lesion, color, location, fish behavior): I have 1 black angelfish. He's acting very strange recently. At first, he was swimming still in one corner of the tank with his fins drooped down. He wasn't eating that much as well. Few days later, some white films accompanied with his left and right fins falling off developed. I immediately did research and all symptoms seem consistent with fin rot, though I'm not entirely sure. After medication, his condition is still deteriorating badly. He's now lying and swimming upwards (like an arrow pointing to the surface). He's also weak at times.

Volume and Frequency of water changes: Once every 4 weeks. 50% change. If things get too bad at times, 1 full change a month.

Chemical Additives or Media in your tank: Only some vitamins and dechlorinator. I've been using them for years.

Tank inhabitants: 1 snakeskin gourami, 1 platinum angelfish (only around 2-3 inches tall)

Recent additions to your tank (living or decoration): The platinum angelfish. Quarantined for about a week or two.

Exposure to chemicals: None
 
I would start with why is your nitrate zero. Surely there should be a reason. I'd relate the issue to poor water quality. I'd switch to a waterchange everyweek instead of monthly. There's no reason to do a 100% water change too. It's abit bad for fish in doing so.
 
I really don't know why the nitrate says zero. That's what the testing kit says.

Whatever it is, it's only the black angel that's getting affected by all this.
So a weekly water change? How much? 50%? 25%?
Anyway is there any way for him to get better? He really is in bad shape.

Yes, 50% water changes a week is far better for the fish and if you have the time to do it I would do so.

As for the angel, if his fins have dropped off there's little you can do. He's going to have trouble swimming and controlling himself. If he eats he may end up just living with it if not I would suggest putting him down but a lot of people dislike this.
 
Please see if you can provide a picture, it could be any number of things. Some things may clear up up if water quality is alright but some may not.

He's kicked the bucket and moved on to fish heaven.

I got home from school today and found him floating lifelessly on the surface. His fins were nearly gone and there was white film on his eyes, fins, and body.
