Is it sticking out on both sides? From the redness, it definitely looks severely irritated and/or infected. Start on a course of antibacterial foods. If it is sticking out on both sides, it's not something I've seen before.

So it is sticking out of both sides. Could it have swallowed a piece of gravel and it's not able to pass it? I'm not sure what else would cause this issue. Is there a fish vet nearby where you could bring the fish for an x-ray or exam?

Looks like tumor to me unfortunately, for more pics of fish tumors for a comparison, see this link;

There's not much you can really do about such things- if the looks in pain/discomfort from the problem, consider euthanising it (personally this is what i would do at this point since the fish will not get better if its a tumor, , it looks pretty massive already).

I wouldn't bother with separating it, tumors are not infectious in fish just like they are not in people (at least as far as i am aware), they are relatively common in goldfish as far as health problems go because a lot of goldfish that get sold in petshops do not come from very good breeding stock and most tumors or cancers goldfish get seem to be genetic more than anything else.
It will probably be happier with its buddies than in isolation either way :thumbs: .
