Help!

Tank size: 23l (5.5 gallon) Fluval Edge
pH: 7.8
ammonia: <0.15 ppm
nitrite: 0 ppm
nitrate: 0 ppm
kH: Hard water (don't know specifically)
gH: -
tank temp: room temp (approx 20-25 degrees celcius)

Fish Symptoms:
Fish a) CPD appears to have developed a severe deformation. Its stomach is receding and its back is bent. Fish has become more sluggish, probably finding it hard to swim. This has occured recently.
Fish b) CPD has a large, white and red spot on one side. Could either be a tear in its side, or a growth. As far as I can tell, it looks like the latter. Fish behaves normal (compared to the other healthy CPDs): shy, swift when bothered, slow moving when content.

Volume and Frequency of water changes: 20% change every 3 days

Chemical Additives or Media: Water conditioner added to fresh water during water changes.

Tank inhabitants: 7 CPDs, cherry shrimp and assassin snails. Also present is an unwanted damsel fly larvae, about 1cm in length (possible culprit for fish b?)

Recent additions to your tank (living or decoration): Assassin snails were added 2 weeks ago, and a plant, something like an Anubias? (Not familiar with plant names).

Any ideas?

It's a shame to see this happen, but I've been unlucky with my CPDs so far. The Fluval's lighting is too weak to really appreciate their appearance, they hide whenever I come near (even after a couple of months), and I've already lost one CPD when it tried jumping and got caught between tank lid and the rim. The shrimp have always been as happy as Larry.
