This is usually a sign of suffocation. Sometimes from low oxygen levels, but it can also be caused by poor water quality or illness - nitrite prevents the blood from taking up oxygen, and any disease effecting the gills will reduce oxygen intake.
How are your water stats? What's your tank size and stock list? How long has it been running? Any recent additions? How did you handle cycling?
