Something Is Seriously Wrong

Do you have well water, or a municipal water supply that you are filling the tank from after water changes? What are you using to condition the replacement water with when you're doing a water change?

Wilder is on the money with excessove gravel vaccing in a newer tank. You should also let the filter media be for at lest 3-4 weeks in a newly set up and cycling tank. Both the frequent vaccing and filter maintenance are probably disturbing the nitrufying bacteria that are trying to establish themselves.

Oh I didn't mean I clean out the filter every time I do a water change.. only meant that I've only cleaned it in that water.
I've only cleaned the filter out 3 times now.. 1 recently was just to get out the carbon... first time was b/c I read you were supposed to do it so did it the next day. Then the 2nd time was when I had an outbreak of planaria.

I'll lay off the gravel vac for a week to see if anything happens.
Also tap water is just city water.
Just tested it and no nitrites, no nitrates & a reading of 0.25 ammonia as it's had since the first time I tested it.

I was using Top Fin tap water dechlorinator, but got some Stress coat stuff this time b/c the place we went to doesn't carry Top Fin products.

All fish seem to be doing fine though. Cory takes to hiding in one of the decors during the day, but comes out and swims all over the place once the lights are off. Danios are acting like their usual selves.

I'd think about lowering the temperature a little; 80 seems a little high. The warmer the water, the less oxygen it will contain. Aim for around 76-77, but lower it gradually over a few days.
