sick betta help!

my male betta started sinking 4 days ago. he can still swim enough to get to the surface but it seemed to be a struggle so I moved him to a 1/2 gallon tank with a heater to make it easier. my first thought was constipation. we had a heater stop working and before I noticed it the temp had dropped to 70. so I fasted him for 3 days and then tried please with no luck and yesterday freeze dried daphnia. he ate a few and had a bowel movement today. however he does no seem to have improved at all and now he won't eat anything. he still responds to my voice and doesn't seem worse but he is no better. what should I look for next? his scales aren't sticking out, no white shoot, and no fin for. the underside of his head seems a little pale but I may just be trying hard to see something wrong. please help.

Has it got a heater? What is it set to?
Has it got a filter?
How strong is the current?
Is it cycled?
How often do you change water?
How much water do you change?
Do you vacuum the substrate?
What additives or chemicals do you use? Eg De Chlorinator?
Do you use tap or bottled water? If bottled water please give us the brand.
Do you have a water test kit?
Can you tell us the readings for ammonia, nitrite and nitrate?
When you clean the tank can you tell us exactly what you do.
What do you feed him? how much? how often?
Any tank mates? What sort how many?
How long have you had the Betta?
Has he got a history of illness?
Any plants or decorations? Please give details?
Can you provide a close up photo of the fish?

Can you please answer the following questions.

How big is the tank? normally he's in a 10 gallon but when he started sinking I moved him to a hospital tank that's 1 gallon. I used water from his tank so he wouldn't be shocked.
Has it got a heater? What is it set to? yes there is a big heater in the normal tank and I put a small heater in the hospital tank. I keep it at 80.
Has it got a filter? the 10 gallon yes, the hospital tank no but I've been doing daily 10-20% water changes.
How strong is the current? in the 10 gallon the current is light, the plant leaves don't move at all.
Is it cycled? I cycled his 10 gallon for 2 weeks before I put him in.
How often do you change water? 10 gallon about every 10 days I do a 20% change. 1 gallon daily
How much water do you change? 20%
Do you vacuum the substrate? every time I do a water change
What additives or chemicals do you use? dechlorinator and a fertilizer for the plants.
Do you use tap or bottled water? If bottled water please give us the brand. tap water
Do you have a water test kit? no I usually take it to the pet store but honestly I haven't had the water tested in a few weeks.
Can you tell us the readings for ammonia, nitrite and nitrate? I can't remember exact from last time but I'll take some water to the store today for testing.
When you clean the tank can you tell us exactly what you do.i use a gravel vac and work through the gravel until I'm not pulling detritus anymore. every other or every third change I pull the decore and rinse that and vac under it and lastly I pull the filter and rinse it in the tank water I've pulled out.
What do you feed him? haikari pellets how much? 3-5 how often? daily
Any tank mates? no What sort how many?
How long have you had the Betta? 14 months
Has he got a history of illness?none at all
Any plants or decorations? Please give details? java fern, driftwood, a rock, and a tiny amount of java moss.
Can you provide a close up photo of the fish?
