You are in a very bad situation David. You do not have any information about your tank or its chemistry. From that I must assume that you do not have a reliable test kit to give us that information. The simple two water changes in such a long time means to me that you have allowed your water quality to become significantly degraded. I'm sorry if that seems a bit harsh but it is how I see things. Your best course of action now is to start doing regular water changes of perhaps 30% each week. After a few weeks, you could try adding a few fish, but not very many, at a two week interval. Once you have built your fish population to near typical stocking levels, not the ridiculous levels you see at the LFS, you can stop to consider where you have gotten with our fish.
You have said that you did everything you should do with a water change but please check this against your water change standards. All water changes require that you use a dechlorinator to ensure that all new water is free of chlorine. All new water changes must be temperature matched to the existing water in the tank. That means that you at least check the water temperature with your hand and find a decent match. Any water change is done with adequately oxygenated water. If you have a well system, you use a storage tank that lets the water absorb oxygen, if you have a typical water supplier, this is not a problem. You never let your tank water deteriorate significantly between water changes so the change in mineral content from a water change is not too great.
If you have water samples that show any ammonia or nitrites, you must do a water change to remove those chemicals. The samples you cite are samples that indicate that you had a need for a large water change way back when those samples were done. By now you are probably not facing the same levels of contaminants, but your prior readings do tell us why you lost most of your fish. Now you are facing the simple fact that regular water changes are needed on any tank. As long as you take that to heart, you will have better results when you look at your chemistry. There is really no reason to lose any fish to chemistry problems ever.

ohhh, you really should water change once a week, not every 3 or 4 weeks, oldman has given some good advice :good:
