Plant Problems. Need Serious Help

Below are a few pictures of a sword plant I'm having trouble with.

1. What kind of sword plant is this exactly?

2. What the heck is wrong with it? What is it missing? I have a Flourish tab right under the root ball.

3. there are some newer leaves that are in good condition. Should I prune the older, transluscent leaves off? If I do this, there will only be about 3 leaves on this plant. Is it still a good idea to prune the bad leaves off?

you havent' said what fish (if any) you have. A plec/BN could have done that damage...

That to me is a serious lack of nutrients. I had the same thing happen a while back. The root system on those are huge... when I pulled it out, it had roots 14" long. Maybe put a few tabs in other areas of the tank?

Deffinatley snip those dead leaves off. The plant will spend alot of energy on trying to save the leaves which could make the healthy leaves start to die as well. Cut them off as close to the substrate as possible.

also... what how much light do you have on the tank (and what kind)?
 
The only fish like what was mentioned above are some oto cats. They spend a fair amount of time on the leaves but I don't think they did this damage. I have a few other tabs in other areas. When I last looked, the roots were about 6" long. It's a 55gl tank with 110wts total of light, 2x40wt and 1x30wt tubes. 1x40wt is 6500k, 1x40wt is 5000k, and 1x30wt is 10,000k. I'm attempting DIY Co2 but, success is minimal at this stage but, it's only a couple of days old. I have been dosing with Flourish as well but, I read these are root feeders and thus, the Flourish tabs. Anything I'm missing?
 
OK, I snipped off the leaves at the substrate. Also, what kind of sword is this? It was sold at the lfs generically as "sword plant"
