Neon Tetra Dead!

Can anyone help me why my neon tetras keep dying? pH is 7 and ammonia and nirite is 0

did it have any lumpy body, gasp for air, swim funny? what fish is it in with?

did it shoal or did it hide away by itself?

Has the red stripe gone pale or look bleached out.
