Not sure what to do.. water parameters are okay, all the other fish seem fine. A week or so ago I noticed what appeared to be some damage to the side of the fish. I thought it was maybe he hit something in the tank and tore his skin a little. He seemed fine up to a few days ago then it started to look bad and his face got puffy. He's barely moving now, and mostly has been sitting on the floor and against objects in the tank. Please advise what I should do.

I'm sorry, I am a beginner to fishkeeping and cannot help that much. But he looks in very bad condition and I think you should euthanize him.
What it could be:
Open sore
Wound or injury from being attacked(are there other male gouramis in the tank?)
Was hurt from a sharp object in the tank
Don't listen to any of my advice yet, wait for the experts.
@itiwhetu , you are an experienced fishkeeper. What do you think?

Who else is in the tank? Do you have another tank where this guy can go? It looks like a war wound to me.

I have two tanks, one is a ten gallon with a few neon tetras in it.

The other is a 30 gallon where he is that has some zebras, another guarami, a bristlenose pleco and some corys. Each tank also has some snails and a few amano shrimp. So far nobody has been picking at him or bothering him. I have a fairly large net, around 5''x5'', I can net him and set it at the top of the tank so hes isolated but still in the water. I also added some stress coat. Do you think that would help?

If he was mine I would be putting him in with the neons. Get him away from all the action in the bigger tank. Then just monitor him.

While I was waiting for your reply I decided to net him at the top of the tank, he is now upright and I put a few flakes of food in and he is eating. I really appreciate the advice, I am glad its not a disease. I will remove any sharp decorations that may be in the tank, some of the wood and rocks may not be appropriate for the tank.

The gourami iridovirus can cause sores to appear on the body and the fish becomes weaker before dying.
