I'm not expert, but I don't think it'd be advisable.

what about if i start to treat the sick fish as soon as i introduce the new fish??

I wouldnt add any fish if there's a known risk of Ich in the tank, even if you're treating it at the time. Remember, disaster happens quickly in saltwater and its not worth the gamble to put new fish in a tank that may have an ich infection.
