It's possible he got spooked or someone was playing tag with him and he ran into it.
Total freak accident.

He's in a good place right now and he'll be safe going forward
I think he's gonna be just fine, given some tlc.

and you might want to bury that plant base under the gravel or take it out so nobody else goes and does the same silly antic.

Yeah, please don't beat yourself up over this. It definitely doesn't solve anything. I'd say maybe coat the bottom of the plant base with aquarium sealant or something of that sort. Sparkly clean water will be good for him, and I'm sure he'll enjoy having a tank to himself. Hang in there, you. Not your fault at all.
