Exactly. This tanks is about curing the fish and separating it from the others, not about looks. 

what about bottom dwellers? just throw in some clean sand IF you ever get sick bottom dwellers like cory catfish?

No. The bottom dwellers will be fine with a glass bottom. It's smooth so won't irritate them. They will also hide in the PVC.

I've sprayed the underside of my tank with a stone look paint - three coats of that and a top coat of black gives the illusion of substrate where a pure glass bottom can be unsettling to the fish (depending on the stand you have it on of course).
 
Just be careful not to make it look too good

How would I keep a hospital tank cycled if I wanted to keep it running while not in use?
Should I just have a little guppy or something swimming around in it and remove the guppy when you need it for a sick fish?

You can dose ammonia do keep the bacteria alive.

I don't run in when it's not in use.  I keep a spare filter sponge in a cannister filter.  That allows the bacteria to colonise the filter media for when you need to use it.  The tank gets cleaned down in between uses.
