Help Neon Going Crazy!

I have a school of 12 Neons and one of them is going crazy by speeding around the tank and looking exhausted and dead then going back to normal. My other neons are fine. Water levels are normal.

people tend to need more information to help you,
1. Is you tank cycled?
2. What are you ammonia, nitrite and nitrate levels in PPM?
3. What other fish are in the tank?
4. How big is you tank?
5. How long has it been set up?

There will be some more stuff but can't remember it ATM
 
My ammonia level is 0. But all my other fish are normal just the one. Tank is fully cycled for a year.
15 gallon with 12 neons 3 guppies and some guppy fry and one ghost shrimp.
 
I have never had experience with this, but do you think its neon tetra disease? I am not sure what it is, but maybe its a possibility. Sorry wish I could help. :/
 
Is it swirly and swimming in twisty directions, sort of cork screw-like?  Or is it rubbing or flicking itself on objects?
 
It was just swimming very fast in straight lines. But now it just died today. I don't think it was NTD because it was completely normal exept for the fast swimming and looking dead for two seconds then going back to normal. :(
