Well I have guppys and mine had internal parasites never had tb.
I would get that parasite med as you have nothing to lose as they carry them anyway, thats the best advice I can offer you.

Guppies will no longer keep them, as can't find a breeder of them and the lfs are always deseased.

Hi

I phoned Maidenhead aquatics this morning and the manageress, who was very helpful suggested I take the poorly fish to show her.
She seemed to think the same as you that it could be a parasite or TB.
She kindly offered to euthanase the fish for me.
So she suggested Flubenol which I have treated the tank with tonight.
All the other fish look fine and are eaing well so fingers crossed.
