is it okay to use fw meds in sw tanks?

I wouldn't unless it says otherwise on the outside... And If you have Liverock and/or inverts/coral definitely no meds in the main tank.

So how would i treat a fish if i have no qt tank?

Depends what you have in there. FO or FOWLR or reef?
If it's not FO, then you can always set-up a quick hospital tank, it would be best to isolate the fish anyways.
