Fish head wound help !

Hi all, I need help. I have a threadfin Acara and he is doing bad. He’s got some kind of disease . It started with small sore and now it’s affected his eye and big part of his head. I have treated with anti fungi medicine but no success . Now treating with fin rot and ulcers ( by myxazin) and can not see improvement yet. It’s been second day with this treatment.
He is 2 and a half year old , other fish doing good, water changes once a week about 50% .
0 amonia
0 nitrite
20-40ppm nitrate
Ph about 8.0
Tank is bout 3 years old 600 litres.
I feel like he is in pain and that makes me very sad ?
I have added pictures to how it started and how it looks now. It’s been around 2 weeks that he is like that. Please help me to identify disease and what is the best treatment to use .

Thank you , I’m dealing with this first time too. I’ve been in fish hobby for 12 years and never seen this before. Tried to google but I can’t find nothing similar. I just feel the pain for him ?

Yeah the first image looks a minor , I’d of guessed a scratch or a fight but the way it’s developed it looks like it’s slowly rotting the fish away , does it seem like it’s suffering ? Still eat swim ect

At the start he was eating but now he is not. I feel like it was a wound because my pleco sometimes can get aggressive with food and sometimes chases Random fishes. He could have hit something . I just don’t know ? I believe he is in pain . It looks like he is being eaten by this rott. And it started to affect his eye. It got bigger and cloudy. He might loose his eyesight.

Thank you. When should I see an improvement? It’s a third day of treatment and it’s says use it for 5 days. Do I need to use longer if I don’t see improvements ?

Can you not take him out of the tank put him in his own tank or bucket with an air stone keep it dark?

I’d try Kanamycin if it’s not too late. It’s a stronger antibiotic. I usually use it along with Furan-2 (treats bacterial) together. These have been very effective for me.

No I don’t have anything like that nor separate air stone. I just can’t see him recovering . Looks to bad

there’s barely any life left in him. It’s bleeding and I guess it affected his brains . Not much breath here. ? upside down laying on the bottom.

normally you should see an improvement within 24 hours or using a medication. if there's no improvement after a couple of days the medication isn't working.

Thank you . Sadly he passed away. Hopefully won’t have to deal with something like this again . Had him for over 2 years and I been in tears when he died.
