Where they say to "change the 3D pleated carbonized cartridge once every two to four weeks" they mean to take it out and replace it with a brand new one right? Not just take it out, rinse with water siphoned during a water change and replace?

Yes, that's what manufacturer means when they say "replace". That's because replacement cartridge is where they make all the money. Filters are cheap, but replacement cartridges are expensive. Don't bother with them though. Aquaclear and Penguin/Emperor series filters are far superior to Liberty.
 
I am a long time fan of Eheim. I use nothing else. Until your post, I'd never heard of this range however. They don't sell them here, I asked in a couple of the lfs so I could look at one. Odd.
