The fish has a problem with its brain (as mentioned above). It can be caused by a bacterial, viral or protozoan infection, or by a physical injury. There is no cure and the fish should be euthanised asap to put it out of its misery.
The rest of the fish in the picture appear healthy and there were a couple of male black phantom tetras that were displaying, so they appear to be in good condition.
Monitor the remaining fish and if anymore develop the same symptoms in the next month or two, then it is probably an infection in those particular fish.
The best way to prevent these types of diseases is with good tank hygiene. This includes big regular water changes, gravel cleaning the substrate, cleaning the filter once a month, and not letting gunk build up in the tank or filter. Apart from that, getting healthy fish, which is just luck of the draw.
