I don't have one and I was wondering if the rest of you do. I am planning to get one soon, depending on if the rest of you think it's necessary.
 
I have one. I'd personally recommend it, as I've saved my main tanks 3 times from the outbreak of ich. :)
 
What all needs to be put in there, just a heater, filter and light? Do I need to do a fishless cycle?
 
Filter, Heater, and Light.

It dosn't have to be fancy. For ease of use, I use a filter that uses the same media as my main tank. This way it forms an instant cycle. I just used the established media from the old tank. You can use a simple internal filter though if your budget is tight.

You will want a Light so you can observe the fish. You know what the heater is for of coarse.

You want the tank to have a bare bottom for ease of cleaning. I also through in some pvc pipe for hiding spots. You could also use plastic plants for those plant loving fish.
 
The tank that I would buy is a cheapo from wmart. The filter doesn't use the same media as my big tank. So, should I do a fishless cycle with ammonia? Also, would the tank remain cycled if there were no fish in there?
 
I have a 25 ltrs (8 gallons) which I keep as a hospital/quarantine tank when I finally get round to buying more fish (haven't bought anything since my initial shopping expedition when I set up the tanks 9 mths ago). Atm I keep a couple of convalescent fish in there, but they can be moved.
When I get big tank in the autumn (Rio 240) I plan to move my female platies into there (after fishless cycle) and use the freed-up 15 gals as a quarantine tank.
 
I don't have one a the mo. I plan on upgrading when we move this year and then I'll either turn my ten gallon one into one, or get another.
 
I do have a quarantine tank, although I trust my main LFS as they never mix stock from different suppliers and also quarantine new stock for a couple of weeks before sale.

I would only use it for sick fish that I already owned as I'm a bit dubious about the benefits for new fish - additional stress making them ill. Plus I think that the majority of cases of ich that are blamed on new fish introductions are actually ich in the tank, but the new fish is stressed and therefore is the first to show symptoms.

Probably totally wrong, but it's my theory nonetheless! :D

Oh - almost forgot. My quarantine tank is currently home to Dudley whilst I try and track down those fully equipped betta / pico tanks we were talking about last year (y'know, light, heater, filter included...?)

I would get one. I have a few different 10s, 20s or 29s that could be used as quar tanks with a little stock switching. It doesn't have to be a tank, I saw 18 gallon heavy duty Rubbermaid containers on sale last week for 5 bucks. Worse things have been used for keeping fish.
