Give the tank another water change - minimum 50% and see what happens.
If he shows improvement then do another water change of another 50%.
It is sounding to me that ammonia is the cause of this new problem.
Do you have a liquid test kit? If so what are the water parameters?
Is the tank well oxygenated?
