PH depends on the fish, I wouldn't worry about it being 7, I think all of my tanks are lower than that. If its only on one side I would suspect a tumour, how long has he had it?

a week and a half and then it went way down and last 2 days (post bloodworm day) it went way back up over night. It has been two days at this size.

Once a week, 30% and then 50% with filter changes about every 3 weeks

I agree. Looks like a tumour to me, too. Maybe a cyst, if it changes size and shape?
