It doesn't sound like gill flukes...

Another question...did you acclimate the puffer to that salinity or did you introduce him from the bag to that salinity. Chances are, because most LFS are irresponsible and don't have the resources to have seperate tanks, the puffer was in freshwater at purchase. Correct me if I'm wrong.

I'm just confused because you said you just got him and he's already in a bit of salt.
 
Also, your water is a bit too salty for what the figure 8 likes
