    To me the worms look like detritus worms which points to uneaten food in the substrate as AB suggested. IMO most of the issues are caused by waterquality-issues caused by this and meds won't solve this on the longterm.

    Over the years I have read tens / hundreds of topics alike. Strangely mainly and C.aeneus show these spots on the base of the dorsal. Recently a C.paleatus. Don't know the cause but could be diet related somehow. There is no reason to worry though.

    It is a secretion from a gland behind the dorsal fin. Nothing to worry about and no treatment needed. Quite common in C.aeneus.

    Sounds like they are hungry. Leave cumcumber, sweet pepper, brussels sprouts or something. Also remember they are omnivores that's why they gi for the pellets.
