It does look like ich but it is odd the spots aren't falling off. What is the temp raised to and what meds are you treating with? Any other other fish in the tank and are they being effected as well?

I've had preset heaters die on me and one never stopped heating the tank. I tested it by filling a mason jar and set it in there. Came back a bit later and the jar was hot! I prefer adjustable heaters now but they are rather hard to come by for smaller tanks, and they can be quite pricey.

Can you post some pictures? She could be egg bound in which case the result is often death. Adding epsom salt might help but if she's already at the point of acting lethargic it might be too late. Without pictures I can't say for sure though.

Either way, no fish should behave like then even if they are full of fry. The deformity is likely the cause of her laying on her side. If she happens to drop fry theirs a good change some may have a similar deformity, in which case you should either keep for yourself or cull.
