Help! My fishtank is cloudy and my pleco is distressed

My fishtank is very cloudy for over a week despite partial water changed and trying accuclear, ammo lock and new filters. Not sure what else to try. My pleco is clearly distressed and has an open wound on its nose. Other fish seem okay but he is my 6 year old and priority of the tank. Any suggestions would be awesome! Thank you in advance

Looks like bacterial bloom.Heterotrophic bacteria that use oxygen. I'd aerate the water.

Did you test the water (using chemicals?)
 
Lose the Accuclear. This works by making the really fine particles clog up into bigger particles, so filters can pick them up.
You can imagine the effect on fish gills. ;)

I see a tank full of plastic and no real plants. Whilst I accept plecs will trash planted plants, even stuff floating around would be of huge benefit.

I'll also suggest Aquarium Salt to help the equilibrium of the fish, at a dose of 1 tbsp per 5 gallon. (Dissolve the salt in a jug of aquarium water, before adding to the tank).
