What is wrong with my Tetra?

My black phantom tetra has been like this for a month now no clue of what's wrong with him I've tried different types of medication many water changes no luck. He is eating and swimming normal. If I can get an awnser to this situation will be very grateful thanks

What medications did you try and how long did you use each one for?

Do you have carbon in the filter, because it removes medication from the water?

I have carbon foam in my filter from the ones the tank (Juwel) came with but I've used other medication for finrot in the past and healing medication and they have worked. Here are the filter pads I use. And the medicine that I used was eSha 2000 for fungus and bacteria

I would take the black sponge out and don't replace it. Carbon is not needed full time.
