What Happened!

Two days ago I found my poor humbug damselfish dead in the sand with a huge part of his body missing (gill area). At the time I just had a ocellaris clown and cleaner shrimp in there. What could have done this? I was completely baffled by it, the powerhead and skimmer have holes far too small for anything to get into. Do you think the cleaner shrimp has targeted him?

Could be that he died and THEN scavengers had a bite... This is often more likely than predation by another tankmate

Maybe yeah, it was a pretty nasty hole in it, just happened overnight =[
