What Happened To My Pleco?

This is the first we have seen of our pleco missing his skin.I've had them for about four years. I never had a problem with him until now. Well I did notice his eyes bulging but I added some AquaSafe Balance and they returned to normal.​

I don't. I'm going out of town this weekend where they have actual pet stores instead of Walmart. I'll pick one up then.
