Template for tank info

I had read somewhere on here about a template for questions @Colin_T asked for before treating the tank with salt. Where can I find such a template? It would make giving the answers easier for me

It's pinned to the Tropical Fish Emergencies section under the title "Start Here With your Emergency". However, a lot of the questions aren't always needed. A lot of people copy & paste the questions, then add their answers.
