Sick Kuhli Loach?

I have a Black Kuhli Loach that I recently noticed was sick. I don't know what is wrong and it is my first Kuhli. He has been swimming around but now he is resting in the corner of the tank. I think I had a recent bacterial bloom but I'm not sure. I have searched the past couple of days and I am afraid he has the beginning of Columnaris.... Please help me!!

Aquarium Details:
  • 45 Gallon Tank
  • 4 Dwarf Gouramis
  • 5 Platies
  • 2 Kuhli Loaches
  • 3 Apongeton
  • 2 Amazon Swords
  • 1 Java Fern
 
The fish is covered in excess mucous, which is caused by poor water quality or something in the water irritating the fish.

Check the water for ammonia, nitrite, nitrate and pH.

Do a 75% water change and gravel clean the substrate every day for a week.
make sure any new water is free of chlorine/ chloramine before it's added to the tank.

DON'T add chemicals and DON'T add anti-biotics. Just do water changes.
